Objective To discuss the presentation and management of deep aggressive angiomyxoma of vagina. Methods Aggressive angiomyxoma is a rare mesenchymal neoplasm typically located in pelvis and perineum and most frequently in a premenopausal age group. In this report, we present the case of a patient who had grade 3 cystocele and imaging was suggestive of fibroid uterus. Results Intraoperatively, the mass was found in space of Retzius. Immunohistochemistry confirmed the diagnosis of aggressive angiomyxoma. The mass was excised with resolution of symptoms, and in view of positive margins, local radiation was advised followed by aromatase inhibitors. Conclusion Aggressive angiomyxoma should be considered as differential diagnosis in a premenopausal lady presenting with pelvic mass. Complete surgical resection with negative margins should be the aim followed by consideration of hormonal treatment. Close follow-up is required to detect early recurrence. Ausführliche Beschreibung
