La cofradía de la Santa Cruz no tuvo papel alguno que desempeñar en relación con la salvaguarda de la reliquia hasta hace relativamente poco tiempo. Durante la Edad Media, la cofradía simplememente no existía. En el momento de fundar la institución ya habían transcurrido más de 250 años desde que el concejo de la villa grabara la imagen de la Cruz y la vaca en su sello identificativo, icono que se ha mantenido hasta el día de hoy como emblema de Caravaca.A lo largo de la Edad Media, todo aquello que rodeaba a la Cruz pertenecía a la Orden militar o señorío laíco tenedor de Caravaca en cada momento histórico, incluyendo hasta las propias limosnas que los devotos mandaban a la Vera Cruz.
